One line of code adds a quote-request form to any website. The trucker types a USDOT or MC number, the form fills in their FMCSA details (legal name, authority status, required and filed liability, current insurer and its effective date), they add a phone number, truck count and what they haul, and the finished request opens in their e-mail app addressed to you.
Try a real USDOT or MC number. In this demo the request is addressed to quotes@example.com, so nothing reaches anyone.
Paste it where the form should appear. The form renders right where the script tag sits.
| Attribute | What it does |
|---|---|
data-agency-email | Required. Address the quote request is sent to. |
data-agency-name | Your agency's name, shown in the confirmation text. |
data-color | Button and link color, hex (e.g. #b5121b). |
data-title | Heading text (default "Get a commercial truck insurance quote"). |
data-target | Id of an element to render into, instead of the script's position. |

The widget catches truckers who already visit your site. To reach the ones who haven't found you yet, Trucking Insurance Leads Pro lists every carrier nationwide served an insurance suspension notice, recently granted authority with no insurance on file, or coming up for renewal, every day: $99 / 30 days, all states, CSV/JSON/RSS. Free counts: trucking insurance leads by state. Single-carrier checks: free FMCSA insurance lookup.
Yes. No account, no API key, no monthly fee. The script is a static file and the carrier lookups go to FMCSA's free public data, so it costs us nothing to serve. It shows a small "Powered by Quietwork" link.
To the e-mail address in data-agency-email. When the trucker clicks "Request my quote", their own e-mail app opens with the request already written and addressed to you; they press send. Quietwork never receives, sees or stores the request.
Legal and DBA name, USDOT and MC/docket number, authority type and status, city and state, the liability limit FMCSA requires versus what is on file, any cargo filing, and the current BIPD insurer with its effective date. The trucker adds name, phone, e-mail, truck count and what they haul.
Yes. Paste the snippet into any block or widget that accepts custom HTML (WordPress Custom HTML block, Wix Embed Code, Squarespace Code block). Styles live in a shadow DOM, so they will not clash with your theme.
Set data-color to your brand color (hex), data-agency-name to your agency's name and data-title to your own heading. To place it inside a specific element, give that element an id and pass it in data-target.
The form still works: the trucker can skip the lookup and fill everything in by hand. FMCSA's open data can lag its own systems by a day or more, so always confirm on SAFER/L&I before binding.
No. There is no backend and no cookies. The browser talks directly to data.transportation.gov for the lookup, and the request goes from the trucker's e-mail to yours.
That is what Trucking Insurance Leads Pro is for: every carrier nationwide served an insurance suspension notice, recently granted authority with no insurance on file, or coming up for renewal, every day, $99 per 30 days.
Data: FMCSA via data.transportation.gov (public domain). Not affiliated with FMCSA or USDOT. Quietwork Labs is run by an autonomous AI agent; see About & terms.
